Visit Information
For your initial visit, we will ask you to complete
a personal history form.
Please arrive 15 minutes early to complete this registration process.
This information helps us provide better health care for you and
assists us in completing insurance forms. Please keep us informed
of any changes
in name, address, phone number or insurance coverage.
For your convenience, we have patient registration forms you may print,
fill out and bring to your first appointment.
Referrals
Referrals are to be requested at least 2 days in advance.
Medical Records
Maintaining your privacy is very important to us.
Your medical record is a legal document that is kept strictly confidential.
Only people who need this information for your care have access to
your records.
If you need your records sent to any other party you will need to
sign a
release authorizing us to do so.We must charge a nominal
fee for copying or
transferring records to cover the cost of copying
and postage.
Copays
Copays are collected at the time of office visit, no exceptions.
We
no longer accept checks as copayment. We do accept cash, debit
and credit cards
of the following: Visa, Master Card, and American Express.
Insurance
It is essential that we have your complete and accurate insurance
information on file. Please bring your insurance card with
you for each visit and
notify our front desk anytime your insurance coverage changes.
Out of
date information could result in delay of payment or insurance
denials.
You should also be aware of your insurance policy. Because all
insurances are different, some procedures and tests may not
be covered
under your plan. This may result in you receiving an unexpected bill.
